ABSTRACT

OBJECTIVES@#To analyze the cost-effectiveness of the Morse Fall Scale by clinical big data for nurses in the prevention of falls in hospitalized patients.@*METHODS@#A total of 59 358 hospitalized patients, who came from the Third Xiangya Hospital of Central South University in 1 year before nurses were trained by the Morse Fall Scale, served as a control, including 26 862 males and 32 496 females. While the 66 203 hospitalized patients served as an observation group in 1 year after nurses were trained by the Morse Fall Scale, including 29 881 males and 36 322 females. The time spent by clinical nurses in the fall-risk assessment and the fall number were recorded in the 2 groups, and the relationship was analyzed between the Morse Fall Scale assessment and the clinical ending along with the labor cost of nursing. The cost-effectiveness was analyzed. In addition, the incidence of fall in the observation group was compared between the falling high-risk patients and the non-high-risk patients.@*RESULTS@#The Morse Fall Scale showed that the incidences of fall in the observation group and the control group were 3.39/100 000 and 3.82/100 000,respectively, there was no significant difference between the 2 groups (@*CONCLUSIONS@#The evaluation of the Morse Fall Scale has a certain effect, but it is limited in the prevention of falls in adult hospitalized patients, and the cost-effectiveness analysis is not good. It is recommended to implement the intervention measures for high-risk patients after the assessment, which may improve the management level and efficiency of fall prevention.

ABSTRACT

Objective To investigate the risk of falling and nutritional status in elderly surgical patients, and to assess the correlation between them. Methods Patients aged ≥65 in Department of General Surgery of Beijing Hospital between January and June 2015 were enrolled in this study. The Morse Fall Scale was used to evaluate the risk of falling.Anthropometrics, body composition, and Nutritional Risk Screening 2002 (NRS 2002) scores were collected to evaluate the nutritional status of the patients. The correlation between risk of falling and nutritional status was analyzed. Results A total of 383 patients were included, including 314 cases under 80 (65-79 years) and 69 cases ≥ 80.Patients ≥ 80 years showed significantly lower grip [(24.53 ± 8.09)kgvs.(30.57 ±8.48)kg,P<0.05] and4-meter gait speed [(0.66 ±0.19)m/s vs.(0.84 ± 0.20) m/s,P < 0.05],but significantly increased undemutrition [15.9% (11/69) vs.7.0% (22/314),P < 0.05] and nutritional risk [56.5% (39/69) vs.38.2% (120/314),P <0.05].Altogether 33.9% of the patients (130/383) were at high risk of falling, and the prevalence was significantly higher in patients ≥80 than in patients < 80 [44.9% (31/69) vs.31.5% (99/314),P =0.036].Compared with patients not at high risk of falling,high-risk patients had lower body mass index [(22.33 ± 1.82) kg/m2 vs.(23.76 ± 3.26) kg/m2] and grip [(24.95 ± 8.56) kg vs.(30.72 ± 8.39) kg],but higher prevalence of nutritional risk [46.9% (61/130) vs.38.7% (98/253)] (all P<0.05).Conclusions Eldedy surgical patients have a high risk of falling, which may be related with their nutritional status.Nursing and nutrition intervention should be emphasized in there patients to prevent falling and improve clinical outcome.

ABSTRACT

Objective To investigate the clinical application of Morse Fall Scale by primary nurses. Methods Morse Fall Scale was used to assess the falling risk in 21,378 patients by the primary nurses. The application of the Morse Fall Scale, choices of right time for assessment and the occurrence of falls in patients with risks of falling were evaluated by a self-designed questionnaire. Results Among 21,378 patients, 222 patients were at risk of falling. In terms of the items like the history of falls, more than one diagnosis, intravenous injection, the accuracy of assessment was above 91.6%.In terms of the items like aided walking, gait, cognitive state, the assessment accuracy is below 86.0%.The accuracy of assessment timing selection in the situations like admission, the score by Morse fall scale over 24at admission and the accuracy in choosing right time for assessment was was above 90.2% after transferring to other deparments and falling. The accuracy in choosing the right time for fall assessment was below 67.9% under such conditions as apostasies, and oral administration of drugs probably leading to fall. Conclusions It is acceptable that the primary nurses use the Morse fall scale to assess patients with risky falls and for the right choice of assessment.Attention should be paid to strengthen the nurses training to improve the accuracy of the scale,to make sure to choose right time for the assessment so as to prevent the falls of patients to the greatest extent.

ABSTRACT

Objective To investigate the discriminant validity and relativity of three scales applied in fall risk estimation for senile patients. Methods The timed up and go test (TUGT),the Morse Fall Scale (MFS) and the Berg Balance Scale (BBS) were used by two trained testers to evaluate the fall risk of 161 senile in-patients.The patients were divided into a falling group and a no-fall group based on their history of falling in the previous one year.Student's t-test was applied to compare the discriminant validity of the TUGT,MFS and BBS.Spearman's correlation coefficient was calculated to analyze the correlation among the three scales. Results The scores of patients in the falling group on the three scales were significantly different from those of the no-falls group. The correlation coefficients among the three scales were in the range 0.680-0.888.Conclusion The TUGT,MFS and BBS all showed high sensitivity and good discrimination in fall risks estimation for senile patients. The results with the three scales were highly correlated.Because the emphasis of these three scales is different,a suitable scale should be selected in clinical practice according to the characteristics of the senile patient.
